When Should You Review Your Risk Assessments or HAZOP?

Have there been major changes in your process or plant since the last assessment?
Ask yourself:

Often, changes slip through without proper documentation or evaluation—leading to blind spots in your risk profile.

2. Accidents or Near Misses

Have any incidents occurred that may indicate a failure in existing safety barriers?
Even near misses can highlight critical weaknesses that should be addressed in your next risk assessment.

3. New Projects and System Integrations

Have new projects been added alongside existing systems?
If a team scopes a project in isolation, they may not adequately assess interactions with connected systems. This could introduce unseen risks that should be reviewed comprehensively.

4. Outdated or Incomplete Original Documentation

Does your existing HAZOP or risk assessment stand up to scrutiny?
If it’s outdated or was conducted to a poor standard, it may no longer be fit for purpose and will likely require re-documentation.

5. Loss of Organisational Knowledge

Have there been personnel changes since the last assessment?
If your key team members from previous HAZOPs have left the organisation, you may have lost valuable contextual knowledge—making your current risk assessment less reliable.
